I've never encountered this before. I'm guessing OMS is conflicting with MIDI manager.

Lime will run without MIDI manager, but will not have MIDI capabilities. Apple has pretty much dropped MM development and we soon hope to have MIDI function using QuickTime.

I would try removing all the OMS extensions.
